This report contains our assessment of the medical care provided to incarcerated persons at San Quentin State Prison between October 2021 and March 2022. We rated the overall quality of health care provided at San Quentin State Prison during this time period as inadequate.

This report summarizes the California Department of Corrections and Rehabilitation’s performance in conducting internal investigations and handling employee discipline cases we monitored and closed from July 1, 2022, through December 31, 2022. Of the 184 cases we monitored and closed during this timeframe, we rated 125 cases satisfactory and 59 cases poor.

This report summarizes our monitoring of the inquiries and investigations the California Department of Corrections and Rehabilitation performed from January 1, 2021, through December 31, 2021 into allegations of staff misconduct incarcerated persons raised through the department’s grievance and appeal processes. The report concludes that despite its revised regulatory framework for processing its staff misconduct cases, the California Department of Corrections and Rehabilitation continues to earn poor ratings; flawed practices by departmental staff into staff misconduct allegations cause inquiries and investigations to be deficient.

This report contains our assessment of the medical care provided to incarcerated persons at Chuckawalla Valley State Prison between July 2021 and December 2021. We rated the overall quality of health care provided at Chuckawalla Valley State Prison during this time period as adequate.

In this audit, we set out to determine whether the department’s drug interdiction processes provide effective means of preventing drugs from entering prison grounds, detecting and discovering drugs within prisons, and investigating the sources of discovered drugs. We also evaluated the department’s process for collecting and reporting data related to drug discoveries and performed transactional testing to assess reliability of the data. Finally, we reviewed the data to assess the impact of operational restrictions implemented in response to the novel coronavirus (COVID-19) pandemic, on the introduction of drugs in prisons. In the report, we provide the department with 19 recommendations to improve its drug interdiction processes.
